• Manage a client caseload of 15-20 adults who were deemed incompetent by the court of law and have a severe mental illness diagnosis.
• Facilitate and teach group and individual courtroom procedures and terms.
• Provide and connect clients with resources to ensure stability while out in the community including housing, healthcare, therapies, and/or substance abuse/mental health programs.
• Collaborate with team members weekly such as psychiatrists, physicians, supervisors, and other care mangers to ensure clients are receiving ethical care and are reaching their goals.

• The behavioral health therapist is responsible for performing the psychosocial assessment and assessing the individual’s needs.
• Facilitating therapeutic group sessions: CBT, DBT, psychotherapy’s, process sessions, and life skills development.
• Developing recovery plans with the client and appropriate staff members. Offering safe environments to build rapport with clientele and engage in individual therapeutic sessions.
• Collect collateral from client’s network, friend and family support. As well as advocate for the client’s needs in wants in treatment meetings.
• Perform plans for discharge with the client.

• The waiver support coordinator is responsible for assessing the individual’s needs, preferences and future goals.
• The waiver support coordinator develops person centered support plans, coordinates face to face visits, monitor client services, and facilitates quarterly progress meetings while also assessing client safety.

• This is a professional position responsible for processing intake and placement referrals, assess client needs, place foster children in the least restrictive setting and coordinate services as appropriate within Hillsborough County or with Case Management and or with Child Protective Investigators.
